A case of acute pyogenic thyroiditis induced by acupuncture

Abstract
Thyroiditis is the disease of variable etiologies of which acute pyogenoic thyroiditis is quite rare, and may be associated with pyogenic infections of the other sites of the body.
We report a case of acute pyogenic thyroiditis, which was developed by acupuncture on underlying toxic diffuse goiter.
The patient was 26 years old female and she noted a nodule on right anterior neck incidentally, 8 years ago, and, thereafter she has experienced easy fatigability, diaphoresis, heat intolerance and weight loss. She received acupuncture on the site of nodule for therapeutic trials several times. Symptoms of acute infection, including a fever, local tenderness, and swelling were persistent.
Upon admission, BT was 38.3¡ÆC, and pulse rate was 120/min and regular. On physical exam, diffuse reddish swelling on right anterior neck, tenderness and bruit were found. Laboratory studies showed elevated levels of T3, T4, low TSH and an elevated leucocyte count with a polymorphonuclear predominance. The thyroid scan revealed an area of decreased radio uptake corresponding to the involved portion, and 24hrs RAIU was increased. Treatment was performed with appropriate antimicrobial drugs after identification of the causative agent by needle aspiration.
